Fees must be paid in order to pick up schedules

Thursday, July 25, 2013 - 2 PM to 7 PM --students with last names from A-K

Friday, July 26, 2013 - 7 AM to 10 AM --students with last names from A-Z

Tuesday, July 30, 2013 - 12 PM to 7 PM --students with last names from L-Z
Parents of returning students with questions about the centralized one-stop registration program should call Cathy Thomas at 566-7414. Parents new to the district seeking information about registration can also call Mrs. Thomas.

Students New to the District - Registration Open Registration - New Families are encouraged to register their student(s) as soon as they move into the district's boundaries. Please stop by the school office with required information beginning June 3, 2013 between 8:00 am and 2:00 pm.

Two (2) forms of Proof of Residency
are required for
every NEW and RETURNING student
THE DISTRICT WILL NOT ACCEPT A POST OFFICE BOX NUMBER

First form of "Proof of Residency" should be a
CURRENT UTILITY BILL (gas, water, sewer, electric or land phone bill)

Second form may be one of the following:

Occupancy permit with current date

Current public aid card with current address in district

Driver's license with current address in district

Current voter's registration card with address in district

Lease with last months rent receipt

Additional utility bill such as phone, trash, etc
EXCEPTION TO THE TWO PROOFS OF RESIDENCY: Private housing on Scott AFB. ONE form of proof will be accepted from those living in private housing on base. You will need a signed letter, from Scott Landings, verifying the name of the Military person, rank, date of move in and complete address.

Illinois Health Physical Form
Students new to Illinois must have a physical completed on a State of Illinois Health Physical Form.
All Pre-K, Kindergarten, 6th grade, and 9th grade students must have a recent Physical. (Sports physicals must be done yearly.)

Dental Exam Form A dental examination form completed by the dentist, or waiver, if the student qualifies for one of four criteria, must be completed for students in grades Kindergarten, 2nd grade, and 6th grade. The completed form must be turned in to the school nurse by May 15th of that school year.

Vision Exam Form Vision exams will be required for all Kindergarten students and any student new to the district. (Illinois law requires that proof of an eye examination by an optometrist or physician who provides complete eye examinations be submitted to the school no later than October 15 of the year the child is first enrolled or as required by the school for other children. The examination must be completed within one year prior to October 15 of the year the child enters an Illinois school.)

Vision exams are required for all students entering Kindergarten or an Illinois school for the first time.
